Joint account divorce warning

My husband and I had a joint bank account with Lloyds and when we divorced he kept the account number and I had my name removed. He moved away and I moved house and a few weeks later, my ex husband accused me of hacking into his bank account. A MEMBER of staff had typed my address onto his account without our knowledge as I wrote to them closing a policy. This was sorted. However, I recently opened a joint account with a lady for a houseshare with Lloyds. This account was subsequently closed. Guess what a MEMBER of staff has accessed my ex-husbands account and typed my new address onto his account yet again. How can this happen, why does it happen. I have told my ex to close the account as they are obviously dense. I have had a credit card denied .. my ex has been receiving letters telling him his credit limits are being cut and asking if he is in difficulties.
Summary: close accounts agree to change banks
