I've had a Boots Advantage Card for years now - in fact it was the first loyalty card I ever got. I can honestly say that in my opinion it's the best.
This card offers 4 points per £1 spent in Boots which is the most generous loyalty scheme around (as the ads say!). Each point is worth 1p, which makes it very easy to see how much they are worth - e.g. 1436 points mean you have £14.36 to spend in store. This is probably the most transparent scheme around - other schemes allow you to amass 1000s of points that are only worth about £5.
As well as collecting points on normal purchases (in store and online - but you have to go to a kiosk in store to add points gained online) there are often special offers allowing you to collect, say, 100 bonus points on certain purchases. In addition, a few times a year you receive a little booklet containing money off or extra points coupons - these are often tailored to you personally, based on your past purchases. Owning a card also means you can get a free copy of the Health and Beauty magazine, which contains articles and beauty tips as well as more vouchers.
Boots obviously introduced the scheme in order to get you to spend more money in store. It's worth keeping in mind that if you can find a product cheaper elsewhere, you're probably better off buying it there rather than going to Boots for the sake of the points. However, if the product you want is the same price or cheaper (and it is quite likely to be - I've noticed that Boots seem to have lowered their prices, especially of skincare, in the last few years and are much more competitively priced) then it's totally worth choosing Boots.
Don't forget to check out an Offers Kiosk too, you can print out more money off or extra points coupons, as well as check your points balance and change your personal details.
You can spend your points on anything in store - you can't redeem them online yet. I tend to save them up and buy a luxury makeup item that I couldn't normally afford - last time I got an Urban Decay eyeshadow palette!
In summary - definitely get this loyalty card if you ever spend any money in Boots at all. You'll be pleasantly surprised!
