I never go out without my bag. I actually carry two bags, one is my hand bag and the other is the bag for my kids.
I change my hand bags depending on my outfit and occassion but the kids bag is always the same.

The must have's in my bag are,

cash
credit card
identification card
mobile phone
a pen
a diaper each for my almost 2 year old and 3 month old
lip gloss
pack of facial tissues

And my kids bag which is a lovely green semi circle shaped bag contains,
a change of clothes for each child
diapers for each child
wipes
a small towel
some snacks
bottle, water and formula
and my 3 month olds soother if its not in her mouth already

My hand bags are usually the shoulder type bags with one strap. I've got many hand bags but prefer the ones which are on the bigger side so that I can stuff in more items.

Summary: If you have a big bag you will surely carry atleast one thing you don't need
